To cancel or modify your membership, you must complete and sign an official bodyworks massage cancellation/modification form available at the front desk BEFORE you want the cancellation or modification to become effective.

If you cancel your membership DURING the 12 months, you will be charged a cancellation fee of $150. There is NO cancellation fee if you cancel after the first 12 months.

Failure to pay for the 2 consecutive of the membership will automatically terminate your membership and all remaining massages will be forfeited.
